The Premium Care Package provides advanced, skilled nursing oversight and hands-on care for clients with complex medical needs who wish to remain safely at home. Services are delivered by a licensed Registered Nurse and are designed to support medical stability, reduce hospital readmissions, and improve overall quality of life through coordinated, person-centered care.

Comprehensive nursing assessment and ongoing reassessment
Development and oversight of individualized plan of care
Chronic disease management (diabetes, CHF, COPD, hypertension, etc.)
Pain assessment and management coordination

Medication reconciliation and organization
